Facility: Throgs Neck Rehabilitation and Nursing Center

Location: The Bronx, New York

Schedule: Full-Time | 9:00 AM - 5:00 PM

Salary: $70,000 - $75,000

Throgs Neck Rehabilitation and Nursing Center is seeking a compassionate and dedicated Social Worker to join our team in The Bronx, NY. This full-time role is essential in supporting residents’ emotional, social, and psychosocial well-being while ensuring compliance with state and federal regulations. Key Responsibilities

• Develop, implement, and evaluate comprehensive social service programs for residents
• Maintain current knowledge of federal and state regulations; recommend policy updates as needed
• Participate in discharge planning, resident assessments, and individualized care planning
• Conduct interviews with residents and families to obtain psychosocial histories and identify support needs
• Coordinate transportation and connect residents with appropriate community resources
• Educate residents and families on Medicare, Medicaid, and available financial assistance programs
• Collaborate closely with nursing, medical staff, and the interdisciplinary care team
• Maintain accurate, timely, and thorough documentation and progress notes
• Support quality assurance initiatives and assist during surveys and inspections
• Participate in key committees, including Infection Control, Policy Advisory, and Quality Assessment
• Ensure end-of-life care decisions (MOLST/POLST) are properly documented and honored

Qualifications

• Bachelor’s or Master’s degree in Social Work (required)
• Minimum of 1 year of experience in a social services role
• Nursing home or long-term care experience required
• Strong communication, organizational, and problem-solving skills
• Ability to work effectively in a collaborative, team-oriented environment
