The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Job Overview:

Work with a team of transplant professionals to provide care in a multidisciplinary setting. Perform psychosocial evaluations on pre-transplant patients to assess their feasibility to transplant. Provide ongoing assessment of the patient and families ability to handle this complex procedure and ability to provide care. Facilitate support groups and community education. Work with community organization to provide for the patient’s psychosocial needs.

Qualification

Required:

  • MSW from accredited University
  • LSW Required

Essential Functions:

  • Assist in the placement of transplant patients to long term care facilities or rehab. Provides training to such facilities as necessary to meet patient needs.
  • Ensure compliance with UNOS, JCAHO and other regulations as it relates to psychosocial care of the transplant patient. Works closely with departmental leadership to ensure compliance.
  • Facilitates placement of transplant patients to sub-acute or nursing home care. Works closely with hospital case managers and social workers to ensure smooth processes between departments.
  • Interacts with all hospital departments to identify opportunities to better meet patient needs.
  • Interview patients and families in order to complete a comprehensive psychosocial evaluation. Ensure intake for meets all dept and regulatory requirements.
  • Maintain knowledge base of community, private industry and/or government agencies that can assist in meeting patient’s needs.
  • Provide education to patient and family as it relates to transplant or other chronic disease states.
  • Provide ongoing documentation of patient and family status in the patients chart. Periodically audits charts of other social workers to ensure compliance with documentation requirements and identify opportunities for improvement.
  • Provide problem focused counseling to those patients in needs.
  • Works closely with transplant financial coordinator to ensure patients understand coverage for transplant surgery and post transplant care. Develops a smooth and efficient process for reviewing pt coverage status.
  • Ensure all inpatient and outpatient needs are met in a timely manner.
